VNA TeleCare™ is an exclusive service of
Visiting Nurse Association Healthcare Partners of Ohio (VNA), which has
been providing quality home healthcare for more than 100 years. Since 2002, VNA
TeleCare has provided in-home telemonitoring to more than 600 patients. Today,
there are over 150 monitors in the Northeast and Central Ohio region. Our VNA
Advanced Practice Nurses monitor up to 14 different vital signs and are able to
prevent repeat hospitalization for many patients.
The VNA is a family of independent nonprofit agencies offering
a continuum of services, including:
VNA Cleveland provides state of the art,
high-quality home healthcare to individuals and families throughout Northeast
Ohio. Services include skilled nursing, rehabilitation, mental health and
personal care services.
VNA Mid-Ohio delivers the same high-quality,
compassionate home healthcare services as VNA Cleveland to individuals and
families in Central Ohio.
Hospice and Palliative Care Partners of Ohio offers
special support for those with terminal illnesses: hospice, pain management,
symptom control services and a variety of support services.
VNA CarePlus - Private Duty - provides
skilled nursing and personal care assistance by the hour, by the shift, or
around the clock.
VNA HouseCalls- In-Home Physician Services -
delivers primary medical care and consultation to persons who are homebound or
have difficulty getting to the doctor’s office through a team of VNA Physicians
