Everyday, VNA TeleCare reminds you when it’s time to check your vital signs; including blood pressure and blood sugar. It also asks you simple questions about your appetite and energy level to get a comprehensive look at your health. It’s a pretty remarkable machine, but not nearly as remarkable as the people who watch over you.

For you or your loved one with a chronic medical condition
VNA TeleCare is ideal for patients with chronic illnesses and other serious conditions that require frequent monitoring— cardiovascular disease, respiratory illness, diabetes, hypertension and many others.

Easy and Simple
TeleCare requires minimal technological knowledge to use. VNA TeleCare is so easy to use, a patient who can make a telephone call can use VNA TeleCare.

Fewer emergency room visits and hospital admissions
VNA TeleCare patients have fewer emergency room visits and hospital admissions than patients with similar medical conditions who do not use the monitor. Why? Because the TeleCare nurse can identify changes in vital signs before they become bigger problems. The VNA nurse is able to consult with your doctor and intervene before a hospitalization is necessary.

TeleCare is free for VNA patients
VNA TeleCare is free to qualified patients of the Visiting Nurse Association. There is no equipment purchase or rent. No set up or maintenance charges. No monthly fee. And as a patient of VNA, you are eligible for a wide array of other convenient home healthcare services that can keep you safe, more comfortable and healthy at home.
